Please use the format shown below for your grant application. Each page of the grant application should display the organization name and a page number. An application may not be accepted if it fails to follow formatting requirements or if it lacks any of the information requested below.
GRANT APPLICATION
The grant application should not exceed five (5) pages and should contain the following sections and information.
Summary (Use this format for the Summary)
1. Organization Name, Address, Telephone Number and Website/Email
2. Year of Incorporation
3. Name, Title, Phone Number, and Email of Contact Person
4. Name and Title of Person Authorized to Accept and Sign Grants on behalf of Applicant
5. Name of Project
6. Requested Grant Amount
7. Name of Fiscal Agent (if applicable) and Name and Title of Person Authorized to Accept and Sign Grants
Organization
Provide a brief and succinct history of your organization, including its mission, population served, as well as qualifications to accomplish the project.
Project Description
Briefly describe the project and its scope. Include goals or objectives, anticipated and measurable outcomes, and timeline of activities. (The project period will end no later than December 31, 2023.) Explain how your project fits with The Strong Foundation's funding focus. Discuss any collaborative partnerships that are part of the project.
Project Budget
List expenses, administrative costs, income projection, funding sources for the project budget. Indicate whether funding is secured or pending. In addition to the project budget, provide a condensed version of the total organization operating budget details, excluding expenses and funds pertaining to land acquisition and capital improvement projects. If the project and organization budgets are the same, please indicate this. Explain the intended uses of the grant funds, i.e., specific project expense items or general operational support. Please do not attach separate Excel files.
List of Board Members
Tell us who are your organization's Board Members. Please provide both their names and affiliations. The list of Board Members should not exceed one (1) page of the grant application.
SEPARATE ATTACHMENT
In addition to the 5 page grant application, please submit the IRS documentation. Although the application and the IRS letter are forwarded together, they should be separate files. Please refer to the "Grant Process" page for more information.
